Job Responsibilities
- Conduct comprehensive patient assessments and work collaboratively with the interdisciplinary care team to create, implement, and update individualized care plans.
- Identify patient needs and initiate referrals to appropriate healthcare disciplines, or clearly document clinical justification when referrals are not required.
- Support patient health, safety, and independence through education, coaching, and rehabilitative interventions, guiding patients and caregivers in effective self-care practices.
- Provide clinical supervision and guidance to Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs) to ensure quality care delivery.
- Maintain accurate, timely, and compliant clinical documentation in accordance with organizational and regulatory standards.
- Communicate patient status, progress, and changes in condition regularly to the clinical manager and interdisciplinary team.
- Assist with planning and delivering staff training, in-services, and educational programs as needed.
- Participate in on-call rotation and provide on-call support and care to patients and families as assigned.
- Engage in ongoing clinical development and participate in required continuing education and professional growth activities.
- Perform additional duties as assigned to support patient care and organizational goals.
