Job Responsibilities
- Provide direct personal care assistance to patients in their homes, supporting comfort, hygiene, and daily living needs.
- Utilize proper safety practices and body mechanics when performing personal and supportive care tasks, including ambulation, transfers, positioning, range-of-motion activities, and light household duties.
- Plan, prepare, and serve nutritious meals, and assist patients with feeding as directed.
- Perform assigned homemaking and environmental services to maintain a safe and clean living environment.
- Observe patient condition closely and promptly report, document, and communicate any changes or concerns to the primary nurse or supervising clinician.
- Follow individualized, written care instructions developed by a Registered Nurse in accordance with the established plan of care; ensure instructions are received prior to providing services.
- Assist patients with prescribed exercises and therapeutic activities as instructed by the nurse or therapist.
- Conduct and document simple urine testing for glucose, ketones (acetone), and albumin as required.
- Carry out additional duties as assigned to support patient care and agency operations.
