Job Responsibilities

  • Conduct comprehensive patient assessments and work collaboratively with the interdisciplinary care team to create, implement, and update individualized care plans.
  • Identify patient needs and initiate referrals to appropriate healthcare disciplines, or clearly document clinical justification when referrals are not required.
  • Support patient health, safety, and independence through education, coaching, and rehabilitative interventions, guiding patients and caregivers in effective self-care practices.
  • Provide clinical supervision and guidance to Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs) to ensure quality care delivery.
  • Maintain accurate, timely, and compliant clinical documentation in accordance with organizational and regulatory standards.
  • Communicate patient status, progress, and changes in condition regularly to the clinical manager and interdisciplinary team.
  • Assist with planning and delivering staff training, in-services, and educational programs as needed.
  • Participate in on-call rotation and provide on-call support and care to patients and families as assigned.
  • Engage in ongoing clinical development and participate in required continuing education and professional growth activities.
  • Perform additional duties as assigned to support patient care and organizational goals.
